KidsPark is a parent cooperative and requires an annual registration process. New members are welcome anytime throughout the year. Registration materials are available below or you can pick up copies at KidsPark. Please remember you must read the Parent Handbook, return fully completed registration forms and pay the registration fee prior to your child's first reserved day. Your medical forms(Health Care Summary and Immunization form) MUST be completed by your doctor's office and be received by KidsPark before your child's first reserved days. These forms may also be faxed to KidsPark at 651-642-0724. Note: The Minnesota Department of Health provides the Immunization Form and it states that for children older than 15 months a health care provider signature is not required. However, KidsPark licensing and KidsPark policy requires a health care provider signature for all children, including those older than 15 months.
Returning families must re-register by submitting the yearly membership fee along with new contracts, parent information forms, emergency cards, and co-op information form. New health care forms and immunization records must be completed by your health care provider for each returning child. Forms from the previous year will not be accepted.
